Procedure and Terms of Rendering Health Services

Commercial health services are provided by JSC «Medicine» (clinic of academician Roytberg) to the patient under the following terms:

upon demonstration of a personal ID document,
upon filling out and signing the personal data consent form,,
upon signing the Agreement for commercial health services rendering (outpatient, inpatient, etc.),
upon filling out the Request for service provision (diagnostic or laboratory),
upon signing the Appendix to the Agreement for commercial services rendering—the invoice for services,
upon payment for the selected services.

The Agreement, the Consent and the Invoice can be formalized with any administrator-cashier on any storey of the Clinic.

To obtain commercial health services, please contact any administrator-cashier at any storey of the Clinic.

The health services can be paid for by a third person who will receive a certificate for tax deduction. Please take care in advance to provide the administrator-cashier with the payer details (the payer should be present during payment for the services) so that he or she could enter the information in the calculation program. If the payment is made with a plastic card, no person but its holder can be registered as the payer. Payment procedure

Payment for the services can be executed using one of the following methods:

cash payment with any administrator-cashier;
card payment with any administrator-cashier;
payment by bank transfer to the settlement account of the clinic (upon receipt of the proforma invoice in the contracts department). If the payer is a legal entity, the contract with the legal entity and the tripartite agreement for payment on behalf of the patient are required;
via the Internet for the selected service;
via the Personal Account on the Clinic’s website;
via the online banking system;
by receipt of a credit from the bank represented in the territory of Meditsina Clinic JSC.

Roszdravnadzor operates under supervision of the Ministry of Health of the Russian Federation and is guided by the Constitution of the Russian Federation, Federal constitutional laws, instructions of the President of the Russian Federation and its Government, international agreements of the Russian Federation, instructions of the Ministry of Health of the Russian Federation, and the Provision “On Federal Service for Healthcare Supervision” approved by decree of the Government of the Russian Federation No. 323 dd June 30, 2004.
