The Center of Gynecology of JSC "Medicine" (clinic of Academician Roitberg) applies an integrated approach to the diagnosis and treatment of diseases of the female reproductive system, using both conservative and surgical methods. Conservative gynecology is aimed at the prevention and treatment of diseases, functional disorders and reproductive problems without surgical intervention. However, in cases where conservative therapy does not bring results, we use advanced surgical techniques.


Operations are performed in two main ways: through the natural genital tract and using incisions. The first method includes hysteroscopy and hysteroresectoscopy, which make it possible to visualize and treat various gynecological conditions such as polyps, fibroids and endometriosis with minimal intervention.


For the treatment of prolapse of female organs, which is often found in postmenopausal women and is accompanied by urinary incontinence, restoration of the anatomical structure of the pelvic floor, including labiaplasty, is performed.

1. Operations through the natural genital tract

Hysteroscopy and separate diagnostic curettage of the walls of the uterine cavity and cervical canal

During the procedure, an optical device (hysteroscope) is inserted into the uterine cavity through the vagina. A camera is attached to it, which transmits enlarged images to the monitor. The doctor can visually assess the condition of the tissues, carefully inspect the necessary areas by enlarging the picture. This method is used in the diagnosis and treatment of various gynecological conditions, for example, endometrial polyposis, uterine fibroids, endometriosis, glandular cystic endometrial hyperplasia, cervical cancer, etc. During the procedure, an endometrial scraping is taken for histological examination in order to determine the cause of the pathology. The method allows you to remove endometrial polyps, perform a medical abortion, and dissect adhesions.

Hysteroresectoscopy

This is a modern gynecological procedure in which surgical intervention does not violate the integrity of the uterine walls. The treatment is based on electrosurgical or laser methods of tissue removal under visual control. Video systems are used to view the organ cavity during surgery.

Indications for hysteroresectoscopy are:

  • Submucous fibroids (located under the mucous membrane).
  • Menometrorrhagia (abnormally prolonged/profuse bleeding from the genital tract during menstruation). Surgical treatment is indicated in the absence of the effect of hormone therapy
  • Endometrial hyperplasia in case of ineffectiveness of drug therapy or if there are contraindications to it.
  • Ascherman syndrome — intrauterine synechiae (adhesions).
  • Polyposis or some types of single polyps.
  • A septum in the uterine cavity.

Prolapse

This is the omission or prolapse of the female genital organs, which are a fairly common disease. They occur in women of all age groups. Postmenopausal women are more likely to suffer from prolapse. The consequence of the pathology is urinary incontinence and constant irritation in the perineum area.

Surgical treatment allows to eliminate anatomical defects of the pelvic floor – the restoration of the vagina and perineum is carried out. The young group of patients includes those who were injured during childbirth and experience discomfort.

Plastic correction also includes labiaplasty.

2. Operations requiring an incision

In this case, there are two types of surgical access: classical abdominal and laparoscopic or endoscopic. In the first case, an incision of less than 10 cm is made, the second implies minimal punctures of 0.5 to 1 cm in size on the anterior abdominal wall, through which instruments are inserted.

Laparoscopic operations are more gentle compared to traditional surgical methods. The main advantage of laparoscopic surgery is minimal intervention and a shortened recovery period, which takes only a few days. Due to its low trauma and high efficiency, laparoscopy is now recognized as one of the advanced techniques for the diagnosis and treatment of various diseases, allowing patients to return to a full life faster.

Laparoscopic conservative myomectomy and hysterectomy

Laparoscopic myomectomy or hysterectomy is an effective method for the diagnosis and treatment of uterine fibroids in the early stages. The use of modern technologies, such as the latest suture material and advanced antibacterial therapy, allows not only to safely remove the tumor, but also to preserve the uterus, providing a woman with the opportunity to maintain reproductive function. Laparoscopy for ovarian cyst

Ecyst is a benign formation that forms in the ovaries. Its appearance depends on the type of cells it consists of.

  • Follicular cyst and corpus luteum cyst appear when the follicle is actively maturing. They do not require surgical intervention and disappear on their own.
  • Serous and mucinous (epithelial) tumors arise from cells of the upper layers of the ovary.
  • An endometrioid cyst appears from a focus of endometriosis that has entered the ovary area.
  • Dermoid cysts (teratomas) are formed from remnants of embryonic tissue in the ovary.

The formations listed above can reach large sizes and cause pain, disrupt the function of the ovary. In this case, they are removed using laparoscopy, carefully separating them from healthy tissues in order to preserve the functions of the ovary.

Treatment of cervical pathology

For the diagnosis and treatment of the cervix, we use high-precision methods, including radiowave surgery, which provides painless and accurate removal of the affected tissues. This is a surgical procedure in which the affected tissue of the cervix and cervical canal is removed. Such an operation is necessary for the diagnosis and exclusion of malignant tumors.

Hymenoplasty

Hymenoplasty is a procedure for restoring the hymen, which can be performed both in the short and long term. Short-term hymenoplasty is an express operation with minimal discomfort and blood loss, the result of which lasts up to 14 days. Long-term recovery provides a more durable reconstruction that can only be destroyed mechanically, for example, during sexual intercourse. The operation is fast and efficient, takes no more than 15 minutes, and the full recovery period is up to 7 days.

Infertility treatment (reproductive surgery)
